Transform each input element into a
SourceTOut, TMat of output elements that is
then flattened into the output stream by concatenation,
fully consuming one Source after the other.
Emits when a currently consumed substream has an element available
Backpressures when downstream backpressures
Completes when upstream completes and all consumed substreams complete
Cancels when downstream cancels
